Post-Holiday Return to School Rapid Antigen Test Instructions

STEP 1: Create an account on Primary Health for your child(ren) using the link below:
https://my.primary.health/l/mount-tamalpais

STEP 2:  On Monday, January 3rd have your child take the rapid antigen test.

STEP 3:  If one of the following is true, the antigen test must be followed up by a PCR or Molecular test and notify covidresponse@mttam.org:

1. The rapid antigen test was positive
2. The antigen test result was negative but your child has COVID-like symptoms. Student must stay home until a negative test is confirmed by PCR or Molecular test.  COVID like symptoms are:
   1. Fever of 100.4 or more
   2. Chills
   3. Cough
   4. Shortness of breath
   5. New loss of taste or smell
   6. Sore throat
   7. Diarrhea
   8. Runny nose or congestion
   9. Body ache
   10. Fatigue
   11. Headache
   12. Nausea
   13. Vomiting

STEP 4:  Log onto your Primary Health account (you should have a link once you are registered in step 1) and report your results.  Please post results if positive or negative.
